Problematic persistent pain is associated with changes in neurophysiology, psychology and overall function, potentially making it difficult to treat and manage. Researchers have therefore attempted to identify factors that predict which patients with acute pain may develop persistent and widespread pains. The studies typically involve large cohorts of patients and statistical findings, but how do risk factors for chronicity apply to the individual and how can they inform our therapeutic approach? In this lecture, I shall review some of the changes that characterise persistent pain, as well as risk factors for its development. I’ll suggest a clinical model that integrates approaches to understanding the transition from acute to chronic pain, broadening our clinical practice to include prevention.

Dr. Hubert van Griensven qualified as in physiotherapy in 1988 and in Chinese acupuncture in 1996. He gained an MSc in Pain at King's College London in 2003, while he wrote a textbook about the application of pain physiology in manual therapy practice. He gained his PhD from the University of Brighton in 2013. In the same year he published his second textbook on pain, with contributions from experts around the world. The latest fully revised edition of this book came out in 2023. Hubert lectures on the Advanced Clinical Practice programme at City St George’s, University of London, where he leads an MSc module on pain. He works in a physiotherapy practice in Northwest London. He relaxes by working with sound, practicing Chinese exercise and enjoying London.
